You'll own a panel of patients through their chronic care journey — check-ins, remote monitoring, and the everyday needs that keep people out of the ER — while co-leading group medical visits that bring our patients together around health, connection, and aging well. This is a hands-on clinical support role for a physician who wants meaningful patient impact without a full-time US schedule.

The Logistics

● Role: Part-Time, Remote

● Hours: 20–25 hrs/week (4–5 hrs/day), US Pacific Time (PST) core hours

● Tech Stack: OptiMantra / eClinicalWorks, Spruce Health, DeepCura (AI Charting), Slack, Google Suite

● Reporting: Reports to Sangeeta Agarawal, DNP, NP-C — Founder & CEO

● Compensation: Base salary plus performance bonuses

What You'll Do

1. Chronic Care Panel Ownership

● Own a defined patient panel for Chronic Care Management (CCM), Transitional Care (TCM), and Remote Patient Monitoring (RPM) — ensuring weekly check-ins are completed and documented

● Be the responsive point of contact for your patients' everyday needs — medication refill requests, symptom questions, and urgent concerns — routing anything requiring a prescribing decision to our US-licensed physicians for same-day sign-off

● Coordinate with Assisted Living, Memory Care, and Home Health partners to close the loop on their requests

2. Group Visits & Patient Engagement

● Co-facilitate group medical visits — presenting content, guiding discussion, and creating a warm, engaging experience

● Manage the participant list end-to-end: enrollment, reminders, attendance, and follow-up

● Support light outreach to grow class enrollment (partner reminders, simple patient invites — no cold sales)

3. Clinic Support

● Jump in wherever the clinic needs an extra hand — documentation gaps, patient outreach, or ad hoc projects — as a true team player

What We're Looking For

● MD degree required (any country of training welcome)

● Experience in internal medicine, family medicine, or chronic disease management preferred

● Excellent written and spoken English; comfortable on video calls with US patients

● Reliable, tech-comfortable, and organized — you keep your own panel on track without hand-holding

● Warm, patient-first communication style; group facilitation experience a plus
